The role of sipping in the nutritional support of geriatric patients

Introduction: Malnutrition is a significant problem in old age. Malnutrition is present in 30 - 70 % of hospitalized elderly and its consequences are severe. Systematic screening and early treatment are essential to ensure and maintain good health in the elderly and are an integral part of overall geriatric care. Nutritional support with sipping - oral nutriti- onal supplements (ONS) is one of the basic approaches in the management of malnutriti- on in geriatric patients. Aim of the thesis: The main aim of this thesis was to evaluate the intake of sipping in geri- atric patients and to monitor possible pitfalls in its use. To answer the main objective, four research questions were set to provide the following answers: 1. What proportion of the total energy intake (CEP) of a patient is from diet and from sipping, and how does gender and age affect energy intake from sipping ? 2. What proportion does dietary and sipping protein make up of the patient's prescribed intake? 3. Will the diet, including sipping, cover the patient's energy and protein needs? 4. Will patients drink their prescribed dose of sipping, and will certain factors such as education by the nutritional therapist and ability to eat affect the patient's in- take of sipping? FRAGILITY AS A TOPICAL NURSING PROBLEM

Introduction: Frailty within geriatrics is still more viewed as a current problem in the world than in the Czech Republic and it is more commonly mentioned. We can say that in the Czech Republic this problematic is still developing, updates and is trying to find its place in nursing as such. Objective: The objective of the presented thesis is to find out if frailty is perceived as a current problematic in the current geriatric nursing. Further, we focused on the geriatric nursing and specifics of nursing care in this field; on what is the awareness of medical staff at aftercare department about geriatric frailty and in which way, within nursing care, they apply their gained knowledge in practice. Methodology and research sample: To achieve the set objective the qualitative research conducted using the technique of semi-structured interview with nurses at aftercare department was used. The basis of this interview was 24 open questions which were predetermined and are documented in attachments (attachment 1) of the thesis. During the interview these questions were extended as needed by additional questions. The interviews were conducted with 10 general nurses at aftercare department of one unnamed South Bohemian hospital. The analysis of interviews was done using open coding paper-and-pencil and the individual information were classified into categories and subcategories. The results grant us comprehensive overview on the nursing care at aftercare departments within the geriatric nursing, utilization of knowledge of nurses about geriatric frailty within their practice and whether nurses perceive the concepts such as geriatric patient, geriatric syndromes and the already mentioned geriatric frailty. It was found out that some addressed nurses at aftercare department of one unnamed South Bohemian hospital never heard some of the mentioned terms and never registered them during their practice either. Other nurses say that they only heard those terms during their college studies, but they also agree that they never met them during their practice. Despite that, all nurses were able to accurately and aptly characterize the terms geriatric patient, geriatric syndromes and geriatric frailty based on their experience and knowledge. Furthermore, it was proven that they realize frailty during the care for patient and can apply their knowledge in nursing care. Conclusion and practical use: The results of the submitted thesis will serve as a template for educational seminar.

Title: Transfemoral prothesis and their importance with geriatric patients Objective: The objective of my work is to gain an overview about transfemoral prothesis and summarize the information about their importance with geriatric patients due to their polymorbid status. Methods: The first part, theoretical, is of finding character, and the second part, practical, gathers information from geriatric patients. Publications, articles and counceling with specialists in prosthetics were used when creating this work. Results: Using transfemoral prothesis with geriatric patients is strongly influenced by their health status and proper rehabilitation. Using of Restraints in the Geriatrics

In consideration of the demographic trends of our population, it is necessary to deal with the old age problems and problems of geriatric patients. The controversial topic of restraints, especially sensitive in association with old frail people, is closely related. The issue of restraints is defined by law. However, the ethical aspects of their application should also be considered. The theoretical part of this thesis deals with the topic of old age and serious problems of this period of life, including mental disorders. It introduces the field of geriatrics and geriatric patients. In further chapters, this thesis discusses the restraints and ethical issues related to their use. By using qualitative research methods, the empirical part of this thesis surveys the attitudes of healthcare professionals towards this issue. It discusses specific restraints and the reasons for which they are used. The results show that the most important value of healthcare professionals is health and the most respected ethical principle is the principle of beneficence. Creatine monohydrate as a part of sarcopenia prevention in geriatric patients

Introduction: Sarcopenia is a disease with a high prevalence in the elderly population and a significant negative impact on quality of life. Physical activity along with adequate nutritional intervention are key to mitigating these effects. Methods: The research was conducted using a questionnaire survey in which 113 nursing homes participated. The quality of nutritional care in these facilities was determined using a survey. Results: A total of 113 nursing homes were included in the research. Of these, 57% of facilities had a registered dietitian available. Nursing homes assess the presence of malnutrition at least 4 times a year in 74% of cases, of which they most often use changes in weight (80 %) and monitoring of food intake (70 %). Only 50 % of the facilities use validated nutritional screening tool. 83 % of the facilities apply nutritional support, most often in the form of diet modification and sipping (98 %). They use food fortifications less often, in a total of 57 % of facilities. The presence of a registered dietitian is affected by the size of the facility, with the larger ones being available to the registered dietitian at a higher frequency. The presence of a registered dietitian in the facility significantly positively affects the frequency of detection of malnutrition (P = 0.039)....

Nursing care of a patient with iatrogenic perforation of oesophagus

The theme of the final thesis is the nursing process in a patient with iatrogenic perforation of the esophagus in the Department of Geriatric Medicine. I consider the chosen topic to be very interesting, but not too often presented from the viewpoint of a nurse and nursing. Based on a single patient case study, the objective was to analyze the care provided for a patient with iatrogenic perforation of the esophagus in the Department of Geriatric Medicine. Nursing care was focused on the care of a geriatric patient with an esophagostomy, mediastinal drain and cognitive deficits. Another goal was to demonstrate that even in the geriatric department, very good results can be achieved by providing a good patient care. The last objective was to compare the procedures provided by nursing care in the Czech Republic and abroad. In my final thesis, I used a case study method, implemented by a nursing case in a patient in the geriatric department and aftercare of a regional type medical facility. Information for the final thesis was drawn from Czech and foreign literature, own observation and provision of nursing care, interview with the patient and from its available documentation. The role of the nurse in the developement of self-sufficiency of geriatric patient.

The aim of this thesis is to describe the role of the nurse in the development of the self-sufficiency of the geriatric patient in the health care facility and in the residential social facilities. In connection with this objective, the main hypothesis has been established: Nurses working in a health care facility perceive the importance of developing self-sufficiency differently than nurses working in social care facilities. The empirical part of the thesis was elaborated using a quantitative research survey, which was carried out using a questionnaire of author´s own design. The questionnaire included questions about nursing perceptions and attitudes to maintain and develop the self-sufficiency of a patient/senior-age client in a hospital and social care facility. The first part was focused on basic identification data, the second part dealt with how sisters perceive the maintenance and development of seniors' self-sufficiency. The next part was to find out how nurses are self-serving in a self-sufficient patient. Further, the questionnaire examined the nurses' efforts to use compensatory aids and their opinion of working with other specialists. The obtained results were processed in MS Excel using graphs and statistical testing. The research group consisted of nurses working in hospital facilities and social care facilities in the South Bohemian Region. The results of the thesis are divided into 5 areas, where the view and attitude of nurses working in individual facilities for the development of self-sufficiency of the patient / client of the senior age is compared. Further, they describe the opinions of the sisters on: the perception of the importance of the development of seniors' self-sufficiency, the influence of self-sufficiency on the quality of the senior´s life, the performance of self-care for the seniors and the use of compensatory aids in maintaining and developing their self-sufficiency. The work provides a general view of the patient/seniors in the field of maintaining and developing self-sufficiency. The results can be used by nurses in practice as well as students of non-medical health care disciplines. It also provides evidence of the importance of maintaining and developing self-sufficiency in providing comprehensive care and improving the quality of life of seniors. The work can be the basis for further research.
